The team targeted the Verbal Fluency Index, a core part of the Edinburgh Cognitive and Behavioural ALS Screen (ECAS), the standard test clinicians use to catch cognitive decline in MND. Their system pairs the WhisperX speech-recognition model with the Silero voice-activity detector and refined timestamping to transcribe recordings and measure pauses, then feeds clinically-inspired features into regression models to predict the score. Tested against systems built on raw acoustic features and self-supervised audio embeddings, the clinically-inspired approach won clearly, hitting an R2 of 0.9 on one fluency subtask and 0.8 on another.\n\nECAS normally requires a trained examiner sitting with a patient, which limits how often cognitive status gets checked as the disease progresses.
