Each hospital's data carries the stylistic fingerprint of its own stethoscope, and the team found that fingerprint gets tangled up with the actual disease signal, so simply stripping it out was unreliable and damaged the underlying content. Instead, BTS-CAFE combines three tricks: causality-inspired interventions that target device-linked style while limiting distortion to content, counterfactual metadata augmentation to blunt shortcuts tied to device and demographics, and gradient alignment across hospitals to push toward decision boundaries that hold regardless of stethoscope. Built on the BTS model paired with CLAP, a language-audio pretraining system, it raised the out-of-distribution ICBHI Score by 3.69 points on average across five stethoscopes the model never trained on, beating standard data augmentation and federated learning baselines on the ICBHI and SPRSound datasets.\n\nThat gap matters because a model trained on one clinic's stethoscope has historically flopped when deployed on another's, exactly the kind of hidden brittleness that keeps AI diagnostic tools stuck in pilot programs instead of wards.
